Clinical Pharmacist, RPh

 

Responsibilities:

Pharmacist will work on a clinical team with other Pharmacists, Pharmacy Techs and Registered Nurses in effort to manage a group of home infusion patients. Pharmacist will have a great deal of impact on patient care. Pharmacist will be responsible for making sure IV type meds are prepared appropriately and sent out for use with home infusion therapy patients. Pharmacist will be responsible for processing new admissions. Pharmacist will help to answer questions and work with nurses and physicians to help monitor care of the patients. The company is an accredited, well established and growing home care company.

 

Requirements:

Requires clinical experience working with IV infusion meds, preferably in a hospital, home infusion, or long term care pharmacy setting.  Requires licensure as a Registered Pharmacist.
 

Must be authorized to work in the United States.
