Team Pharmacist, RPh

 

Shift:
Full-time Monday-Friday days

Responsibilities:
Full-time Pharmacist will work on a clinical team with other Pharmacists, Pharmacy Techs and Registered Nurses in effort to manage a group of home infusion patients. Pharmacist will have a great deal of impact on patient care. Pharmacist will be responsible for making sure IV type meds are prepared appropriately and sent out for use with home infusion therapy patients. Pharmacist will be responsible for processing new admissions. Pharmacist will not have face to face contact with patients. Pharmacist will help to answer questions and work with nurses and physicians to help monitor care of the patients.
 

Requirements:
Requires significant clinical experience working with IV infusion meds, formularies, and injectables. Requires experience in an infusion pharmacy environment, preferably in a home infusion pharmacy setting.  Requires licensure as a Registered Pharmacist RPh.  Must be authorized to work in the United States.
